GETTING STARTED
We would like to welcome you to our office and thank you for allowing us the opportunity to provide you with our best care. In many cases, another physician has referred you to our office. We will work closely with them to see that your needs are met in a most professional and efficient manner.
Historical Records
We ask that all of our patients make sure that any records, diagnostic studies, operative reports and/or radiological films related to the body part being treated, be hand carried to our office. This enables us to evaluate your condition most effectively.
Change of Clothing (as appropriate)
Be prepared to change into shorts or a tank top at your appointment. When possible please bring your own garment to change into. This is will ensure both your comfort and the doctor’s ability to perform a complete examination.
Prescriptions
Prescriptions and refills are only issued during clinic hours. Please call your pharmacist for refills at least 24 hours before you will need the medication. If it is a new prescription that you will need, please call the office with your pharmacy information during office hours. We also require a 24-hour notice. Before leaving home or office to pick up your prescription, remember to call the pharmacy to ensure that it is ready for you.
Release of Information
Prior to release of information concerning your medical records to any third party you must sign a special form authorizing release of this confidential information. Please do not request the clinic to release your records prior to completing this form. Clinic policy requires your attending physician to review records before releasing them to anyone.
